The clinical encounter may be over, but the risk is not.


Health plans, ACOs, health systems, and other organizations carrying accountability for cost, quality, utilization, and patient experience know that outcomes are shaped by what happens beyond the clinical setting.


The Post Visit Gap™ is the period after a healthcare encounter when patients and caregivers must manage the next steps before another reliable care touchpoint occurs.




During this time, people may be trying to understand instructions, manage medications, arrange follow up care, recognize a change in condition, or decide whether a concern requires clinical attention.


The gap can follow a hospital discharge, urgent care visit, specialist appointment, rehabilitation stay, or telehealth encounter. It can also begin when a home health visit ends, approved private duty nursing hours are exhausted, or a Hospital at Home episode concludes.


These services serve important purposes. The Post Visit Gap is not a criticism of them, nor is it a replacement for clinical care, care management, portals, or home based services. It is the space that can remain after a defined service or encounter has ended.

That space matters even more as more people recover and manage health conditions at home. Solo agers, people living alone, family caregivers, and households with intermittent support may face responsibilities that are difficult to see from inside the healthcare system.


Data is central to understanding this risk, but data alone is not enough. Data without context is activity. Intelligence helps leaders understand where to look, what questions to ask, and where attention may be most useful.


No single organization owns the patient. However, every organization involved in care carries responsibility for continuity within its role.
