Why the moments after a healthcare visit deserve as much attention as the visit itself.
As a nurse who has spent more than two decades working across hospitals, managed care, hospice, long term care, primary care, and the pharmaceutical industry, I have seen an important pattern.
For many people, the hardest part of healthcare does not happen during the appointment.
It happens after.
When patients and caregivers leave a doctor’s office, hospital, emergency department, or telehealth visit, they often leave with instructions, medication changes, follow up appointments, and questions that continue long after the visit ends.
For healthcare professionals, the encounter may be complete.
For patients and caregivers, it is often just beginning.
The Post Visit Gap™ can occur across many healthcare encounters and extends beyond traditional transitional care.
An Overlooked Part of the Care Journey
Healthcare organizations have made tremendous progress improving care inside clinical settings. Programs such as transitional care provide valuable support for many patients following hospitalization.
However, questions and uncertainty after a healthcare encounter extend beyond traditional transitional care programs.
They may arise after a primary care visit, specialist appointment, urgent care visit, emergency department visit, telehealth encounter, outpatient procedure, or hospital discharge.
The hours, days, and weeks following these encounters can leave people wondering:
Is this symptom normal?
Did I misunderstand the instructions?
Should I call my doctor?
Did I forget something important?
What should I do next?
These moments can create unnecessary stress, confusion, delayed follow through, and avoidable healthcare utilization.
They can affect patients, family caregivers, care partners, and solo agers who may be navigating healthcare decisions without a built in support system.
I refer to this broader period of uncertainty as the Post Visit Gap™.
